We have current openings for:

Skilled Nurse
The Primary Care Nurse is responsible to provide within a family centered approach, skilled nursing or other therapeutic services. Promotes, maintains and restores the health of individuals and the community through appropriate teaching, counseling, case finding and rehabilitative measures.
​
Qualifications:
-
Graduation from a hospital school of nursing or a university nursing program approved by the National League of Nursing.
-
Registered Professional Nurse with a current license in Connecticut.
-
One year home care nursing experience and/or general nursing experience preferred, but not required.
​​
Physical Therapist
The Physical Therapist slows the progression of or prevents the functional decline and disability of individuals
Qualifications:
-
Completion of an approved program in physical therapy.
-
Current license in the State of Connecticut.
-
Minimum 3 years clinical experience in physical therapy.
​​
Occupational Therapist
Train and instruct individuals on how to use specialized rehabilitation equipment and techniques that improve their ability to conduct daily activities.
​
Qualifications:
-
Completion of an accredited education program in occupational therapy.
-
Current license in the State of Connecticut.
-
Minimum three years clinical experience in occupational therapy.
​​
Medical Social Worker
Provide comprehensive evaluation of psychosocial status as related to the individuals illness and environment.
Qualifications:
-
A Masters Degree in Social Work from a school of social work accredited by the Council on Social Work Education.
-
Three years social work experience in a health care setting.
​​
Home Health Aide
The Home Health Aide provides personal and hygiene care to individuals.
Qualifications:
-
Successful completion of a training and competency program for HHAs approved by the State of Connecticut Department of Health Services.
-
Successful completion of a nurse assistant or nurse’s aide training course approved by the State of Connecticut Department of Health Services and completion of competency evaluation.
-
A practical or registered nursing student who has successfully completed not less than 75 hours of theory and clinical instruction and a competency evaluation.

Speech Therapy
Provide comprehensive evaluation of patient’s level of function and participation in development of the total patient care plan. Identification of patient and family needs for other home health services and referral for same when needed. Participation in case management conference. Instruction of patient, family and other agency health care personnel in the patient’s treatment regime when indicated.

Qualifications:
-
Completion of an approved program in speech therapy.

Current license in the State of Connecticut.
-
Three or more years of clinical experience in speech therapy.
